My appointment diary is held on paper and contains a note of your name, contact information and fees paid. My diary is never shared with anyone unless exceptionally requested by HMRC as proof of my income.

I also have an online diary with your name and the date of your appointment.

Treatment Records: Records of your treatments are kept on paper. When we first meet I will ask you for details of your health and what you want from the sessions. I will then make notes of each session. This information is only used by me in order to offer efficient treatments and to assess your progress. The notes and the information in them will not be shared with any third party without your consent unless there is a legal or medical need to do so.The notes are stored in a locked filing cabinet when not on my person. They will be kept for the requisite seven years and then safely destroyed. For people under eighteen years old at the time of treatment, the notes are stored until the client reaches the age of 25.

Emails: Any emails between us are held on gmail and are deleted when the information has been dealt with. Any personal information is printed and stored with your notes and then deleted, however there may be password protected copies on my home computer, laptop or phone. I delete these emails within a year of your last appointment. This information is kept entirely confidential by me unless required to be shared by law.
